Registered and validated physicians may print out a form to request complimentary samples of Merck products. A form to request complimentary product samples is available for the following products:

When you finalize your request, you will need to print a Complimentary Sample Request Form. This form must then be signed, dated, and returned to Merck via fax or mail using the fax number or address specified on the form.
